Who to Survey?

The trainees that you will be surveying this year for ADD and MCH reporting are those that COMPLETED your program in FY 2009, 2005, and/or 2000. 

ONLY THESE SURVEYS "COUNT" IN REPORTING TO ADD OR MCH THIS YEAR-surveys of other former trainees, while useful to you, are NOT reported to MCH or ADD.

Timeline

• LENDs/LEAHs/PPCs/DBP's: Although we don't have the date for this year yet, MCH provisional reports are typically due in late March/early April.
• UCEDDs: UCEDD annual reports to ADD (which are due July 30) submitted through NIRS will only include surveys received/entered in NIRS by June 30, 2010. If you receive/enter surveys after that date, they WILL NOT appear in your FY2010 annual report.

Standard Report

A standard report in the Trainee dataset called "Long Term Survey Info" will be incredibly useful to you!!  An Admin level of access is required to view this report that generates a table of trainees for survey (i.e., 1, 5, and 10 years post-training) with any contact information in NIRS and the unique login for each trainee.  The report is organized by separation or "terminal" year (i.e., the last fiscal year for which a trainee record exists).  An "X" will display in the appropriate year column to indicate the surveys completed by the trainee.  

To use the "Long Term Survey Info" standard report, select the current fiscal year (i.e., FY2010).  Remember that the check boxes are filters (i.e., if you check both LEND and UCEDD, your search will return only those records that are marked as BOTH, not "either/or.").

Mail Merge & Sample Letter

If you are interested in creating a mail merge from NIRS, please see "FORM LETTER 2" in the NIRS AAA-test center.  Below is a sample letter. 

Dear FORMER TRAINEE,

As a graduate of the NAME Program at the CENTER NAME at the UNIVERSITY NAME, I hope that you will help us continue to improve the program by taking a moment to complete an online survey to help us learn more about the experiences students have had since completing our program.  

This survey should take no more than 5-10 minutes to complete, and your responses will be treated confidentially.  Please login in and complete the survey before DATE.
